MyDental Plans have 9 options, maximums range from $500 to $2000, we use UCRs and fee schedules (depends on the plan), coinsurances are pretty strong as the plan benefit rises, and so forth. If you are looking for a vision and dental benefit plan 3 & plans 5-9 have both. Plan 7 has ortho and LASIK benefits and Plan 9 has Hearing benefits.
